H.F. 2021 Section 1. MEDICAL ASSISTANCE —— CONDITION OF PARTICIPATION 1 —— COMPLIANCE WITH INDIVIDUALIZED HEALTH PLAN. The department 2 of human services shall require that a recipient of medical 3 assistance submit to a health risk assessment and comply 4 with any individualized health plan subsequently developed 5 by the individual’s primary care provider for the recipient 6 as a condition of continued eligibility. The department 7 shall develop guidelines for the health risk assessment 8 and individualized health plans to be used by primary care 9 providers. The department shall submit any medical assistance 10 program state plan amendment or waiver request necessary to 11 implement this section. 12 EXPLANATION 13 The inclusion of this explanation does not constitute agreement with 14 the explanation’s substance by the members of the general assembly. 15 This bill directs the department of human services (DHS) 16 to require that a recipient of medical assistance (Medicaid) 17 submit to a health risk assessment and comply with any 18 individualized health plan subsequently developed by the 19 individual’s primary care provider for the recipient as a 20 condition of continued eligibility.
