H.F. 504 Section 1. TRANSFER OF PRISONERS —— PROVISION OF MEDICAL 1 INFORMATION —— UNIFORM FORM. By January 1, 2012, the director 2 of the department of corrections, in cooperation with the 3 Iowa state sheriffs’ and deputies’ association, shall develop 4 a uniform medical form to be used by all county jails in 5 providing a prisoner’s medical information to the receiving 6 institution when a prisoner is transferred from a county jail 7 to an institution under the jurisdiction of the department of 8 corrections. 9 EXPLANATION 10 This bill directs the director of the department of 11 corrections, in cooperation with the Iowa state sheriffs’ and 12 deputies’ association, to develop a uniform medical form to 13 be used by all county jails in providing a prisoner’s medical 14 information to the receiving institution when a prisoner is 15 transferred from a county jail to an institution under the 16 jurisdiction of the department of corrections.
