or
guardian
using
funds
from
a
pupil’s
individual

account
in
the
fund
shall
submit
an
annual
report
to
the

department
of
education,
on
or
before
September
30
of
each
24
year,
that
contains
all
of
the
following
information:

(a)
The
amount
of
tuition
and
fees
charged
by
the
nonpublic

school
or
other
provider
of
qualified
educational
expenses
27
during
the
immediately
preceding
school
year,
broken
down
by
28
grade
level.

(b)
The
amounts
charged
by
the
nonpublic
school
or
30
other
provider
of
qualified
educational
expenses
during
the

immediately
preceding
school
year
that
are
considered
qualified

educational
expenses,
less
tuition
and
fees.

The
department
of
education
shall
compile
the
annual

reports

pursuant
to
subparagraph
