enrolled

notice,
on

form

by
the

of
the
following:

requirement

and
development
that
includes
the

high-quality,

rendering
or
animation,
or
an

ultrasound
or

real
image

in
subparagraph

if
the
pupil’s
parent

guardian

the
written
request
described
in
subparagraph

(ii)
The
date
or
dates
during
which
the
instruction
in
