1.
A
health
care
provider
who
orders
a
laboratory
test
for

the
presence
of
Lyme
disease
shall
provide
the
patient,
or
the
7
patient’s
legal
representative,
with
the
following
information:

According
to
the
centers
for
disease
control
and
prevention,
9
as
of
2011,
Lyme
disease
is
the
sixth
fastest
growing
disease
10
in
the
United
States.
11
Your
health
care
provider
has
ordered
a
laboratory
test
12
for
the
presence
of
Lyme
disease
for
you.
Current
laboratory
13
testing
for
Lyme
disease
can
be
problematic
and
standard
14
laboratory
tests
often
result
in
false
negative
and
false
15
positive
results
and,
if
performed
too
early,
you
may
not
have
16
produced
enough
antibodies
to
be
considered
positive
because
17
your
immune
response
requires
time
to
develop
antibodies.
If
18
you
are
tested
for
the
presence
of
Lyme
disease
and
the
results
19
are
negative,
this
does
not
necessarily
mean
you
do
not
have
20
Lyme
disease.
If
you
continue
to
experience
symptoms,
you
21
should
contact
your
health
care
provider
and
inquire
about
the
22
appropriateness
of
retesting
or
additional
treatment.

2.
A
health
care
provider
shall
be
immune
from
civil
24
liability
for
providing
written
information
in
compliance
with
25
this
section,
except
in
the
case
of
gross
negligence
or
willful
26
misconduct.

3.
For
the
purposes
of
this
section,
“health
care
provider”
28
means
a
physician
licensed
to
practice
under
chapter
148
or
an
29
individual
who
orders
a
laboratory
test
for
Lyme
disease
under
30
the
authorization
of
the
licensed
physician.
