If a parent, guardian, or legal custodian executes 3 a power of attorney under this chapter, the parent, guardian, 4 or legal custodian shall, no later than seven days after 5 executing the power of attorney, provide all of the following 6 to the clerk of the probate court: 7 (1) Notification that a power of attorney has been executed. 8 (2) A copy of the power of attorney. 9 (3) The name, address, and telephone number of the agent 10 appointed. 11 b. Failure to comply with the provisions of paragraph “a” 12 renders the protections provided in subsection 6 inapplicable. > 13 2.
