A physician performing a vasectomy shall 9 certify in the patient’s medical record all of the 10 following prior to performing the vasectomy procedure: 11 a. That the man has undergone a scrotal examination 12 and a scrotal ultrasound. 13 b. That the man has been given the opportunity to 14 view the scrotal ultrasound results. 15 c. That the man is given the option of hearing a 16 description of the vasectomy procedure. 17 d. That the man is given the opportunity to 18 hear about the complications related to a vasectomy 19 procedure including those related to the failure rate, 20 postoperative infection, perioperative bleeding, 21 postoperative hematoma, and post vasectomy pain 22 syndrome. 23 e. That the man is given the opportunity to receive 24 information regarding vasectomy reversal including that 25 reversal is not always successful and that reversal is 26 costly, lengthy, and rarely covered by insurance. 27 3. A physician who does not comply with this 28 section is subject to licensee discipline pursuant to 29 section 148.6. > 30 4.
