     1  1    Amend House File 2229, as passed by the House, as
     1  2 follows:
     1  3 #1.  Page 1, by striking lines 10 through 14 and
     1  4 inserting <network to a dental plan that sets fees for
     1  5 dental services that are not covered services.>
     1  6 #2.  Page 1, after line 21 by inserting:
     1  7    <4.  Nothing in this section shall be construed as
     1  8 limiting the ability of an insurer or a third=party
     1  9 administrator to restrict any of the following as they
     1 10 relate to covered services:
     1 11    a.  Balance billing.
     1 12    b.  Waiting periods.
     1 13    c.  Frequency limitations.
     1 14    d.  Deductibles.
     1 15    e.  Maximum annual benefits.>
