     1  1    Amend House File 841, as amended, passed, and
     1  2 reprinted by the House, as follows:
     1  3 #1.  Page 8, by striking lines 17 through 20, and
     1  4 inserting the following:
     1  5    <1.  Beginning July 1, 2005, each expansion
     1  6 population member whose family income equals or
     1  7 exceeds one hundred percent of the federal poverty
     1  8 level as defined by the most recently revised poverty
     1  9 income guidelines published by the United States
     1 10 department of health and human services shall pay a
     1 11 monthly premium not to exceed one=twelfth of five
     1 12 percent of the member's annual family income, and each
     1 13 expansion population member whose family income is
     1 14 less than one hundred percent of the federal poverty
     1 15 level as defined by the most recently revised poverty
     1 16 income guidelines published by the United States
     1 17 department of health and human services shall pay a
     1 18 monthly premium not to exceed one=twelfth of two
     1 19 percent of the member's annual family income.  All
     1 20 premiums shall be paid on the last day of the month of
     1 21 coverage.  The department>.
     1 22 #2.  Page 9, line 6, by inserting after the word
     1 23 <member.> the following:  <The department shall also
     1 24 waive the required out=of=pocket expenditures for an
     1 25 individual expansion population member based upon a
     1 26 hardship that would accrue from imposing such required
     1 27 expenditures.>
     1 28 #3.  Page 9, by inserting after line 26, the
     1 29 following:
     1 30    <4.  The department shall track the impact of the
     1 31 out=of=pocket expenditures on patient enrollment and
     1 32 shall report the findings on at least a quarterly
     1 33 basis to the medical assistance projections and
     1 34 assessment council established pursuant to section
     1 35 249J.19.  The findings shall include estimates of the
     1 36 number of expansion population members complying with
     1 37 payment of required out=of=pocket expenditures, the
     1 38 number of expansion population members not complying
     1 39 with payment of required out=of=pocket expenditures
     1 40 and the reasons for noncompliance, any impact as a
     1 41 result of the out=of=pocket requirements on the
     1 42 provision of services to the populations previously
     1 43 served, the administrative time and cost associated
     1 44 with administering the out=of=pocket requirements, and
     1 45 the benefit to the state resulting from the out=of=
     1 46 pocket expenditures.  To the extent possible, the
     1 47 department shall track the income level of the member,
     1 48 the health condition of the member, and the family
     1 49 status of the member relative to the out=of=pocket
     1 50 information.>
