     1  1    Amend the amendment, H=1417, to House File 841 as
     1  2 follows:
     1  3 #1.  Page 4, by striking lines 25 through 33, and
     1  4 inserting the following:
     1  5    <2.  a.  Beginning no later than March 1, 2006,
     1  6 within ninety days of enrollment in the expansion
     1  7 population, each expansion population member shall
     1  8 participate, in conjunction with receiving a single
     1  9 comprehensive medical examination and completing a
     1 10 personal health improvement plan, in a health risk
     1 11 assessment coordinated by a health consortium
     1 12 representing providers, consumers, and medical
     1 13 education institutions.  An expansion population
     1 14 member who enrolls in the expansion population prior
     1 15 to March 1, 2006, shall participate in the health risk
     1 16 assessment, receive the single comprehensive medical
     1 17 examination, and complete the personal health
     1 18 improvement plan by June 1, 2006.
     1 19    b.  The health risk assessment shall be a web=based
     1 20 electronic system capable of capturing and integrating
     1 21 basic data to provide an individualized personal
     1 22 health improvement plan for each expansion population
     1 23 member.  The health risk assessment shall provide a
     1 24 preliminary diagnosis of current and prospective
     1 25 health conditions and recommendations for improving
     1 26 health conditions with an individualized wellness
     1 27 program.  The health risk assessment shall be made
     1 28 available to the expansion population member and the
     1 29 provider specified in paragraph "c" who performs the
     1 30 comprehensive medical examination and provides the
     1 31 individualized personal health improvement plan.
     1 32    c.  The single comprehensive medical examination
     1 33 and personal health improvement plan may>.
