     1  1    Amend House File 565, as passed by the House, as
     1  2 follows:
     1  3 #1.  Page 1, line 11, by inserting after the word
     1  4 <amounts> the following:  <of not less than ten
     1  5 dollars per individual and twenty dollars per family,
     1  6 if not otherwise prohibited by federal law,>.
     1  7 #2.  Page 2, by striking line 32, and inserting the
     1  8 following:  <shall be assessed on a sliding fee scale
     1  9 based on family>.
     1 10 #3.  Page 3, by striking lines 15 and 16, and
     1 11 inserting the following:
     1 12    <e.  Is not currently covered under or was not
     1 13 covered within the prior six months under a group
     1 14 health plan as>.
     1 15 #4.  Page 3, by inserting after line 33, the
     1 16 following:
     1 17    <Sec.    .  MONITORING AND REPORTING REQUIREMENTS.
     1 18 The department of human services shall monitor the
     1 19 effects of the striking of section 514I.5, subsection
     1 20 8, paragraph "m", by this Act, until June 30, 2005.
     1 21 The department shall submit a report to the general
     1 22 assembly annually on January 15, during the period
     1 23 ending June 30, 2005, that includes the reporting of
     1 24 any increased cost of the hawk=i program resulting
     1 25 from the striking of the paragraph described in this
     1 26 section.>
