  1  1    Section 1.  NEW SECTION.  249A.3A  APPLICANT INFORMATION
  1  2 REQUIRED – PUBLISHING OF INFORMATION.
  1  3    1.  An applicant for the medical assistance program shall
  1  4 disclose the identity of the employer of the proposed
  1  5 beneficiary of health care benefits.  If the proposed
  1  6 beneficiary is not employed, the applicant shall identify the
  1  7 employer of any adult who is responsible for providing support
  1  8 to the proposed beneficiary.
  1  9    2.  On or before January 15, annually, the department of
  1 10 human services shall submit a report to the governor and the
  1 11 general assembly, identifying all employers identified
  1 12 pursuant to subsection 1.  The report shall include the name
  1 13 of the employer, the location of the employer, and the total
  1 14 number of the employer's employees and dependents of employees
  1 15 who are enrolled in the medical assistance program.  In
  1 16 compiling the report, the department shall combine the
  1 17 information collected pursuant to section 514I.12 with the
  1 18 information collected pursuant to this section.  No other
  1 19 information regarding program beneficiaries shall be included
  1 20 in the report.  The report shall also be made available to the
  1 21 public.
  1 22    3.  For the purposes of this section, "proposed
  1 23 beneficiary" means an individual who files an application for
  1 24 medical assistance for the individual or the person on whose
  1 25 behalf the individual files an application, including but not
  1 26 limited to a child or other dependent of the applicant or
  1 27 other individual for whom the application is filed.
  1 28    Sec. 2.  NEW SECTION.  514I.12  APPLICANT INFORMATION
  1 29 REQUIRED – PUBLISHING OF INFORMATION.
  1 30    1.  An applicant for the hawk-i program shall disclose the
  1 31 identity of the employer of the proposed beneficiary of health
  1 32 care benefits.  If the proposed beneficiary is not employed,
  1 33 the applicant shall identify the employer of any adult who is
  1 34 responsible for providing support to the proposed beneficiary.
  1 35    2.  On or before January 15, annually, the department of
  2  1 human services shall submit a report to the governor and the
  2  2 general assembly, identifying all employers identified
  2  3 pursuant to subsection 1.  The report shall include the name
  2  4 of the employer, the location of the employer, and the total
  2  5 number of the employer's employees and dependents of employees
  2  6 who are enrolled in the hawk-i program.  In compiling the
  2  7 report, the department shall combine the information collected
  2  8 pursuant to section 249A.3A with the information collected
  2  9 pursuant to this section.  No other information regarding
  2 10 program beneficiaries shall be included in the report.  The
  2 11 report shall also be made available to the public.
  2 12    3.  For the purposes of this section, "proposed
  2 13 beneficiary" means an individual who files an application for
  2 14 participation in the hawk-i program for the individual or the
  2 15 person on whose behalf the individual files an application,
  2 16 including but not limited to a child or other dependent of the
  2 17 applicant or other individual for whom the application is
  2 18 filed.  
  2 19                           EXPLANATION
  2 20    This bill requires that an applicant for the medical
  2 21 assistance program or the hawk-i program disclose the identity
  2 22 of the employer of the proposed beneficiary of health care
  2 23 benefits.  If the proposed beneficiary is not employed, the
  2 24 applicant is required to identify the employer of any adult
  2 25 who is responsible for providing support to the proposed
  2 26 beneficiary.
  2 27    The bill directs the department of human services to submit
  2 28 a report, on or before January 15, annually, to the governor
  2 29 and the general assembly, identifying all employers
  2 30 identified, and including the name of the employer, the
  2 31 location of the employer, and the total number of the
  2 32 employer's employees and dependents of employees who are
  2 33 enrolled in the medical assistance or hawk-i program.  No
  2 34 other information regarding program beneficiaries is to be
  2 35 included in the report and the report is to also be made
  3  1 available to the public.
  3  2    The bill defines "proposed beneficiary" as an individual
  3  3 who files an application for participation in the medical
  3  4 assistance or hawk-i program for the individual or the person
  3  5 on whose behalf the individual files an application, including
  3  6 but not limited to a child or other dependent of the applicant
  3  7 or other individual for whom the application is filed.
