1 4 inserting the following:
1 5 "Sec. . CASE MANAGEMENT PROGRAM FOR FRAIL
1 6 ELDERS.
1 7 1. The general assembly finds that the existing
1 8 case management program for frail elders administered
1 9 by the department of elder affairs is an important
1 10 component of the long-term care system in this state.
1 11 The program emphasizes the independence and dignity of
1 12 the individual while providing services in a cost-
1 13 effective manner.
1 14 2. The purposes of the case management program for
1 15 frail elders include all of the following:
1 16 a. To provide planning, policy development,
1 17 coordination and administrative oversight.
1 18 b. To provide assistance in the form of assessment
1 19 and care coordination under circumstances in which an
1 20 elder or the elder's caregiver is experiencing
1 21 diminished functional capacity or other conditions
1 22 that require the provision of services by professional
1 23 service providers.
1 24 c. To maintain a system that focuses on the
1 25 delivery of home and community-based services that
1 26 emphasize individual independence, individual needs
1 27 and desires, and consumer-driven quality of services.
1 28 3. It is the intent of the general assembly that
1 29 the department of elder affairs in collaboration with
1 30 the department of human services, area agencies on
1 31 aging, advocacy groups, industry representatives, and
1 32 consumers submit recommendations to the general
1 33 assembly by December 31, 2003, regarding the
1 34 redesigning of the case management program for the
1 35 frail elderly including preadmission screening
1 36 methodologies, independent assessment methodologies,
1 37 level of care determinations and ongoing methodologies
1 38 for the coordination, provision, and delivery of home
1 39 and community-based services."
1 40 2. By renumbering as necessary.
