  1  2    #1.  Page 5, by striking lines 15 through 17 and
  1  3 inserting the following:  "health benefit plan, upon
  1  4 request, annually, but not more than three months
  1  5 prior to the policy renewal date, the total amount of
  1  6 actual claims identified as".
  1  7    #2.  By renumbering, redesignating, and correcting
  1  8 internal references as necessary.
